Information for New Clients

Welcome to my practice! Hopefully your stay will be a brief one. I'm never disappointed when clients get the help they need quickly.
In case you want to "get the jump" on initial paperwork, or if you have lost a form I have given to you, the forms you are most likely to need are listed below. To view and print them you will need the Adobe Acrobat reader, which most people have. (You can get it here if you don't have it. It is free.)

 

When you arrive I will ask you to read and sign a statement of confidentiality and office policies. I do this when we meet so that you can ask questions as you read them.

 

If you are being referred by another therapist for a consultation, then you may want to go ahead and fill out a personal history form. It is somewhat lengthy, but important for an effective consultation. Otherwise, you can take it home and fill it out after our first session. It's easier to fill out more completely after we've met, and you're more comfortable with me.
